How can you use this as a boot device? And does it depend on the motherboard? (I assume it does)

Yes, you can use this as a boot device. I boot from it on a fairly old motherboard that has an Intel H97 chipset. I think motherboards based on H97 / Z97 (or later) should probably work. I think Z97,with Intel 4xxx processors (like i7-4790k), was the first Intel CPU and chipset that supported NVME boot.

If you have any chipset older than Z97 (like Z87 or Z77 or basically any old AMD CPU) then I doubt booting will work. I was pleasantly surprised this worked on my motherboard.

Does this card accept 2-sided (chips on both sides so increased height) or just one sided?

Just 1 side, note the "Twin Design" in the first spec.

And just for clarity, it can hold 2 m.2 drives side-by-side.

will this work on a PCIe 2.0 x4?

Yes. The reduced bandwidth of the 2.0 PCI Express slot impacts only the apparent speed of the NVMe device. Expect on 2.0 PCI a decrease from NVMe 4.0 advertised speeds. On a 4.0 PCIe buss the speeds are from 1800 to 3200 depending on the motherboard. On 2.0 expect 1/4 of that speed. A motherboard with a dedicated NVMe slot will generally perform well, even if speed tests show a smaller number than expected.  Don't be fooled by speed test results on your hardware. Do testing of apps that you use every day. I use the JEYI for ease of installation of the NVMe hardware into different motherboards. The performance has been for me the same no matter what the test results deliver (1800 to 3200 on NVMe ASROCK and MSI motherboards with Intel i7 cpu from 2015 to 2018). The MB slot for the JEYI should in theory not share lanes with another PCI express card (like a video card). Search Wikipedia for "PCI Express" to see the speed differences for MB slots PCIe 1.0, 2.0, 3.0. Expect speed 800 to 3200 from tests, which in real apps is 2X to 6X faster than a 2.5 inch SATA ssd.

I'm trying to use it with two (2) WD Black 500GB High-Performance NVMe PCIe Internal SSD - M.2 2280, 8 Gb/s - WDS500G2X0C on a HP Z420 runing Win Pro 10 64 bit. The OS recognizes only one of them. A quick format didn't work so I went with a low format and I'm at 75% after 12 hours (4 more to go probably..) Is this normal? Could it be something wrong with the card?

Does it include a low profile pcie replacement I/O bracket? Trying to use this in my server to install a boot ssd and a proxmox vm ssd, 2 pcie slots in the server and only one accommodates a full size bracket and that is occupied with a tv tuner for plex.

It did not come with low profile bracket when I bought! But you may simply take out the bracket and use your machines' slot cover. It fits in low profile.
But I would like to remind you that the 2nd slot on this adapter is for connecting SATA, NOT NVMe. Only 1 slot on this card is NVMe.

Can I just use one M.2 2280 SSD card or does both slot have to use for it to work?

Yes. Accepts M.2 NVMe 2280. Also accepts M.2 in other lengths. Move the provided screw to a YEYI card location to lock in the other end of the M.2 card.
