Does this card include the antenna needed for it?

Yes, everything is included. The antennae should already be inside your  laptop/mobile device, as this card does not fit into a desktop PCIe slot. So everything you need is included. Just plug in, and attach the antennae, and you are good to go.

Question 2

Well, it's more of a two part question.

Is this compatible inside an Asus Maximus VI Gene motherboard? If so, does this kit include the antenna and the small cable that connects between the antenna and the mPCIe card?

This card is mini PCI Express and the motherboard has the slot for it so they should work together. It does not come with an antenna or cable. This is an OEM item so it is just the card. I am using mine in an Intel NUC. It has antenna wires already.

Will this fit in a Dell m3800 laptop?

I am looking a this card to upgrade the Wifi in 2 different machines and the documentation on both is terrible so I need to ask about compatibility.  I have an Asus K52F laptop and an HP G72-B60US.  Anyone know if these cards will work on these machines?  Thanks.

As best I can tell this should fit the bill for an upgrade for your wifi cards.

I have similar generation and model laptops in my household and just verified that this card fits in them, so I do not see why it would be a problem for your systems.

The image seems to show 2 versions - I need the M.2 for a Intel BLKNUC5i5MYHE NUC; is this the right product?

This is not the correct M.2 version of the product.  This is the MSATA version.

Will this replace my intel Centrino N1030 adaptor in an Asus U56E notebook?

I am having trouble with random disconnects/device disabled.  Tried updating/rollback drivers for card and router.

I run a PC repair business as a side job, and I have dealt with similar issues many times. This can be a fairly complex troubleshooting and repair process.
You will need to check the compatible wireless card list from ASUS first. If it is not compatible, then the system may not allow the PC to boot with the card installed. Manufacturers do this to ensure that the FCC limits are maintained for each model of laptop. The card's FCC ID number (along with the manufacturer & model) is important to match exactly.
This is why it is best to replace a bad wireless card with an identical replacement in most laptop PC's.
Before replacing the card, you should check if your neighbors are using the same wireless channel as your router. If so, then that could explain intermittent connection issues. I use a wireless networking monitoring app on my phone to analyze the wireless environment. It can help you figure out channel interference as well as identify weak signal spots in your house (which can also cause intermittent connection issues).
Make sure you have the latest wireless drivers installed. Try a clean install by uninstalling the current card's driver first. Use ASUS drivers.
Also, check your antivirus software. I know that some (like Zone Alarm) will cause problems like not allowing your wireless card to be activated in some cases. Solution is to uninstall security SW, then install wireless card and/or drivers, then re-install security SW.
